The serum urate treatment target
The American College of Rheumatology recommends a treat-to-target strategy for patients receiving urate-lowering therapy. Serial serum urate measurements help the clinician evaluate whether the plan is reaching and maintaining the intended goal.
Why a single reading can mislead
Serum urate changes with timing, hydration, illness, medicines, and other factors. It may also fall during an acute flare. If gout is still suspected after a low result during a flare, a clinician may repeat the test after the flare settles.
The number never acts alone
Clinicians also consider flare frequency, tophi, kidney function, kidney-stone history, medication tolerance, adherence, and the person’s preferences. Never start, stop, or adjust a prescription based on a website or one home reading.
